Merging PDFs?

I need to merge one page of a 16 page pdf document back in to the original. Basically it's a contract that I signed the last page of and need to email back to the other party. I'd rather not print out all 16 pages just to replace the last one with my signed copy so anybody know how to do this, or have the software to do so?
